Care Management Specialist

What You’ll Do:
- Coordinate discharge care, case management, and counseling for patients experiencing acute health conditions.
- Collaborate with physicians, discharge planners, and treatment teams to develop personalized care transitions and minimize re-admissions.
- Provide post-discharge follow-up, medication reconciliation, and education to ensure successful recovery.
- Support members in accessing providers, community resources, and managing barriers to care.
- Engage in face-to-face interventions when necessary and develop comprehensive care plans in partnership with providers and members.
- Ensure compliance with HIPAA, regulatory standards, and uphold the highest confidentiality and professionalism.
Required Skills:
- Current, active, unrestricted license as an LCPC, RN, or LMSW in Illinois (with reciprocity if applicable).
- Minimum of 2 years of clinical experience and at least 1 year of managed care or health insurance experience.
- Strong knowledge of medical management policies, procedures, and healthcare systems.
- Excellent verbal and written communication skills paired with superior customer service and interpersonal abilities.
- Proficiency with PCs and database systems.
- Ability to work independently, prioritize tasks, and manage complex cases.
- Willingness and ability to travel within the area as needed; valid driver’s license and reliable transportation required.
Nice to Have Skills:
- Experience with multi-state licensing (for nursing licenses).
- Familiarity with community-based resources, transportation services, and home health coordination.
- Previous experience with transition of care services or patient education programs.
- Additional certifications related to healthcare or managed care.
Preferred Education and Experience:
- Bachelor’s degree or higher in a healthcare discipline.
- At least two years of clinical experience, with a background in case management or care coordination preferred.
Other Requirements:
- Willingness to attend onsite training in Downers Grove, IL.
- Ability to occasionally travel for site visits or training sessions.
- Maintain active licensure and stay current with ongoing training and professional development.
