Location: Illinois (Hybrid, work from home and travel required to Joliet, Oswego, etc. so essentially Will and Kendall Counties

Schedule: Monday – Friday, 8:00 AM – 5:00 PM CST
Pay: $38 per hour
Basic Function
Provide care coordination to members via phone and/or in-person visits. Educate on health programs, support care plan adherence, respond to inquiries, and assist clinical operations. Conduct health screenings, assessments, and care planning, including home or facility visits for complex cases.
Key Responsibilities
- Serve as primary contact for members and support clinical operations.
- Conduct outreach, follow-ups, care transitions, and resource referrals.
- Complete health assessments and develop or update care plans.
- Coordinate services, monitor progress, and manage complex cases.
- Educate members on programs, benefits, and community resources.
- Promote engagement and adherence to person-centered care plans.
- Address member and provider inquiries and route as needed.
- Maintain accurate documentation and meet productivity standards.
- Collaborate with care teams and support program initiatives.
- Ensure compliance with privacy and regulatory standards.
Requirements
- RN or licensed behavioral health professional with 2+ years of clinical experience and active, unrestricted license.
- 3+ years of managed care or wellness experience.
- Experience with care management and health education programs.
- Strong communication, analytical, and computer skills.
- Ability to travel within assigned territory.
Preferred
- 3+ years clinical experience.
- Experience in patient education, condition management, or complex case management.
- Bilingual (English/Spanish).
- Relevant certifications or government program experience.
