Job Description:
- Location: Full-time remote (must reside in Illinois, Texas, Tennessee, New Mexico, Montana, Oklahoma.)
Purpose: Outreach to Cigna Healthcare Medicare customers to:

- Complete health screenings (HRA assessments)
- Identify care coordination needs
- Engage customers in internal Care Management programs
Primary Responsibilities
- Telephonic outreach to complete HRA assessments or process mailed/faxed HRAs
- Educate customers on plan benefits and annual provider visits
- Assist with scheduling medical appointments
- Connect customers to case management and community resources
- Address gaps in care
- Route referrals to appropriate care teams
- Escalate customer concerns/issues as needed
- Maintain accurate documentation for regulatory compliance
Requirements
- Strong written and verbal communication; excellent listening skills
- Empathetic attitude and customer service focus
- Knowledge of healthcare system and Medicare regulations
- Ability to identify barriers and navigate resources
- Proficient in computer applications (Outlook, Excel, Word) and data entry
- Detail-oriented, able to multitask and prioritize
- Team player with strong interpersonal skills
- Education: High school diploma required; college degree preferred
- Experience:
- 1+ years in managed care or health services (population management preferred)
- 1+ years in telephone-based processes and workflow management
