$36-38 per hour
Role Overview
In this role, you’ll work directly with members, providers, and internal teams to assess needs, support care plans, and ensure accurate documentation and regulatory reporting. You’ll also serve as a key representative of the organization in member interactions.
Required Qualifications:
- Active, unrestricted license:
- Registered Nurse (RN) with 3+ years direct clinical care
OR - Licensed Professional Counselor (LPC) or Licensed Master Social Worker (LMSW) with 2+ years clinical practice to obtain licensure
- Registered Nurse (RN) with 3+ years direct clinical care
- 3+ years experience in wellness, managed care, or case management
- Experience presenting clinical issues to members and physicians
- Strong communication, documentation, and analytical skills
- Valid driver’s license, reliable transportation, and ability to travel within assigned territory
- Proficiency with Word, Excel, PowerPoint, databases, and web?based applications
Preferred Experience:
- Patient education or condition management
- Transition of Care experience
- Managing complex or catastrophic cases
- Bilingual (English/Spanish)
- Case Management or other healthcare certifications
