Job Type: Contract to Hire
Expected hours per week: 40 hours per week
Schedule: 8am-5pm CST
Pay: $28.50 per hour 

Summary
This position is responsible for conducting home health assessments and engaging members to educate them on available health care programs and personal care plans. The role supports member adherence to care plans, responds to member and provider inquiries, and provides non-clinical support to the Medical Management team to ensure cases are effectively initiated, managed, and closed.

Key Responsibilities

  • Conduct home health assessments and update member records in the system.
  • Perform outbound outreach and follow-up with members regarding their health care and care plans.
  • Educate members on available programs and services to support adherence to individualized care plans.
  • Build and maintain strong member relationships; escalate concerns to the Case Manager as needed.
  • Coordinate non-clinical activities to support case creation, management, and closure within the care management platform.
  • Respond to inbound member and provider inquiries via phone or written correspondence, researching and resolving issues or escalating appropriately.
  • Encourage program participation, including scheduling appointments and coordinating services such as transportation.
  • Complete accurate data entry, documentation, and required correspondence.
  • Meet established productivity and performance standards.
  • Maintain compliance with HIPAA, corporate policies, and confidentiality requirements.
  • Communicate effectively with internal teams including clinical staff, Provider Affairs, Marketing, and other departments.

Qualifications

  • Bachelor’s degree in Social Work or Psychology
  • LVN or LPN with at least 1 year of managed care experience
  • Registered Nurse (RN)
  • 3 years of care coordination experience in a state-managed or waiver program
  • 3 years of managed care systems experience

Skills & Knowledge

  • Knowledge of medical terminology
  • Experience coordinating member medical needs and providing member support
  • Strong customer service, communication, and problem-solving skills
  • Ability to educate and coach members using motivational interviewing techniques
  • Proficiency in Microsoft Office and general PC applications
