Role Overview

The Case Manager RN leads the coordination of a multidisciplinary team to deliver holistic, person-centered care for a diverse health plan population with complex medical and social needs. This role serves as the primary point of contact for members, caregivers, and providers across multiple communication channels (phone, email, text, and online platforms).

Using the case management process, the RN assesses, develops, implements, monitors, and evaluates care plans to support optimal health outcomes across the care continuum.


Key Responsibilities

  • Lead coordination of a regionally aligned multidisciplinary care team, including Social Workers, Dietitians, Pharmacists, Clinical Support Staff, and Medical Directors
  • Conduct comprehensive assessments of members’ health status, psychosocial needs, cultural preferences, and support systems
  • Develop and implement individualized care plans in collaboration with members, caregivers, providers, and community resources
  • Identify gaps in care, address barriers, and drive improved health outcomes
  • Coordinate care across providers, family members, and community-based organizations
  • Arrange and connect members to appropriate resources (mental health, substance abuse, financial assistance, disease-specific services, etc.)
  • Advocate for members and promote self-advocacy
  • Provide education on health literacy, self-management, medication adherence, and nutrition
  • Monitor care plan effectiveness, track progress, and adjust plans as needed
  • Document member interactions accurately and in a timely manner
  • Support care transitions (facility to home or between facilities) to ensure continuity of care
  • Educate members and caregivers on post-discharge care and follow-up requirements
  • Coordinate durable medical equipment (DME) and transportation services
  • Maintain compliance with professional standards, protocols, and performance metrics
  • Participate in ongoing professional development and maintain required certifications

Education & Experience

  • Associate’s Degree or Nursing Diploma required; Bachelor’s Degree in Nursing preferred
  • Minimum 3 years of clinical nursing experience (acute, post-acute, or community settings) required
  • 1+ year of case management experience in a managed care environment preferred
  • Experience managing patients via telephonic and digital platforms preferred

Certifications & Licensure

  • Active, unrestricted Michigan RN license required
  • Certified Case Manager (CCM) required or obtained within 18 months
  • Chronic Care Professional (CCP) certification preferred
