Care Management Specialist II
Pay Rate: $38/hour

We are seeking a highly skilled and compassionate Care Management Specialist II to join our team. As a Care Management Specialist II, you will play a vital role in ensuring seamless transitions of care, providing exceptional support to our patients, and driving improved health outcomes.

Overview

In this dynamic role, you will be responsible for discharge care coordination, episodic case management, and pre-admission/post-discharge counseling for acute conditions. You will establish relationships with patients, identify their unique needs, and connect them with the right resources to achieve optimal health. Your expertise will help minimize re-admissions, emergency room episodes, and ensure that patients receive the highest quality care.

Required Skills

  • Registered Nurse (RN) or Licensed Master Social Worker (LMSW) with current, active, unrestricted license in the state of operations (or reciprocity if applicable)
  • 2 years of clinical experience
  • 1 year of health insurance/managed care experience
  • Knowledge of medical management policies and procedures
  • PC and database experience
  • Excellent verbal and written communication skills
  • Customer service skills and interpersonal skills to engage with physicians, hospital staff, and patients

Nice to Have Skills

  • Knowledge of community-based organizations, transportation, and home health services
  • Experience with case management software and electronic health records
  • Familiarity with the healthcare system and ability to navigate complex care pathways
  • Ability to work independently and as part of a team

Preferred Education and Experience

  • Master’s degree in Social Work (MSW) or related field
  • 2-3 years of experience in case management or a related field

Other Requirements

  • Ability to travel within assigned location (occasional travel required)
  • Possession of a current, valid driver’s license
  • Willingness to work in a fast-paced environment and adapt to changing priorities
